New Patient Registration

register_nowIf you wish to register with this practice you will need to bring your medical card into the surgery. This will need to be signed inside. If you are unable to find your medical card, we will supply you with a registration form to complete.

From then on, you will be registered with this practice and your medical records can than be requested from you previous doctor.

Because this can take some time, you will be offered a new patient medical with one of our Practice Nurses. She will take details of relevant medical history that your new doctor will need to be aware of any repeat medication you are taking.

Please note that you will not be able to request repeat medication until you have seen either the nurse or the doctor.

You will also be given a practice leaflet, which has our telephone numbers and details of the surgery times and clinics at the surgery

Medical treatment is available from the date of registration.  Please contact reception for further information.
