This access is available via your PC or mobile.

Statment of Intent

By 31 March 2016, this practice is able to offer patients who sign up for online access, access to view coded data and test results via Patient Access.  This is in line with the GP Contract and will follow the guidance held within this.

Texting Service

You may have noticed that the practice is sending reminders of booked appointments to all patients.  We hope that you find this service helpful.  If you do not wish to receive appointment reminders, please just text back OPT OUT and we will stop the messaging.  This also gives you the option to cancel the appointment if no longer needed, simply by texting back the word CANCEL.

PLEASE NOTE: This is only a reminder service and we cannot answer any queries by text.

If you are not receiving text reminders, please contact the surgery to check we have your up to date mobile number.

You can also download the MyGP app from your App store, to book GP appointments on your mobile phone.  All we need is to your mobile number on your records and you will be able to see and book any available Doctor appointments.

Online Access to Summary Care Record Information

The practice is now able to provide patients with access to view online, the information held on their Summary Care Record.

This is also accessed at https://patient.emisaccess.co.uk or via patient.co.uk.

However, due to the ongoing implications of access to medical records and the importance the practice places in your medical records confidentiality, this access will need to be granted separately, so even if you are currently registered to book appointments or request repeat prescriptions, there is another form to complete and issues to consider before you go ahead with medical record access -

Abnormal results and misunderstood information.

If we have given you access to your results, you may seen an abnormal result before the doctor has had time to explain this to you.  You may find this distressing.  You may need explanation of an entry in your records, which has been written in medical terms, you may read these at a time when the surgery is closed and this could cause you worry.

Forgotten History

You may see something in your medical history that you have forgotten about and find this upsetting.

Sharing your information with a relative

It is your choice to share your information with a carer or relative.  However it is also your responsibility to keep your medical information safe and secure.  Please consider this before printing out any information.

If you think that you may be pressured into revealing any of your medical information against your will, the practice recommends that you do not apply for online access.

Information about someone else.

If you read any information not relating to you, please log out immediately and inform the practice.

The practice is only making online access available to patients aged 18 or over who are able to give their own consent.  Being able to view your medical record may help you to manage your condition and provide you with information when travelling abroad, however if you print out any information, it is your responsibility to keep this information safe.

It is your responsibility to keep your user name and password safe and secure, if you think that your account has been accessed by someone that you have not agreed access to, you should change your password immediately.

You can withdraw from online access at any time by contacting the practice.  The practice reserves the right to withdraw access to an individual if we feel that this is not being used appropriately.

If you decide to go ahead with online, please contact one of the receptionists who will be happy to provide you with the information leaflet and registration forms.

Please see below a Link to the leaflet that you will be provided with.
